Integrated Health and Wellbeing Support Services

In accord with national and County policy, local health and wellbeing services work together with Primary Care, (provided by GPs and support staff), mental health and social care teams and local communities groups to provide integrated, personalised care delivered closer to home, focussing on prevention and wellbeing as well as health care..


Rapid Response Intermediate Care (RRIC)

Our local team will support you leaving the hospital to recover safely at home. If you are referred to RRIC when discharged, their team of therapists and assistants will provide tailored rehabilitation and care to help you regain independence. They can provide short-term equipment for up to 6 weeks  to support you back into your home environment after hospital stay. They will then help plan your longer term care plan. School Nursing

Our South Bucks team of multi-skilled staff specialise in child health and wellbeing. This includes specialist community public health nurses, community nurses, child health and wellbeing advisors, and school health assistants. They  work with colleagues in the NHS Trust’s safeguarding children’s team and the a team of multi-skilled staff who specialise in child health and wellbeing. Health Visitor Services

Health visitors based at the hospital provide advice and support at home for those in need due to health or social issues. This includes support for families with young children and adolescent youngsters with physical or mental wellbeing and health problems. For example the local Beeches Health Visiting Team, provides support for families with children under five as part of the  Healthy Child Programme, offering valuable support and resources to families. They are partners in the integrated local team providing health and wellbeing support for our communities. Social Prescribing

Based in our local Primary Care Network our Social Prescribing service can help those with social, health and wellbeing needs to find suitable support services and introduce you to community groups offering friendship, activity and assistance. Integrated Neighbourhood Team for Frailty

The team brings together a range of health and care professionals and community support groups for those with complex needs to help them provide integrated support to meet the specific needs needs and preferences of the individual to help them regain their confidence and deal with their health and wellbeing issues.


Community Wellbeing Hub

We are delighted to announce the opening of the Buckinghamshire Community Wellbeing Hub (BCW Hub) at the Chalfonts and Gerrards Cross Hospital. Funded by Friends, the new BCW Hub will provide space for physical, mental health and social care professionals and  community organisations to support activities for the health and wellbeing of our local community in South Buckinghamshire. The first BCW Hub was launched in 2023, and to date, they have co-designed and co-delivered community wellbeing hubs based at Buckinghamshire New University (BNU) campuses in Aylesbury and High Wycombe. The hub began as a partnership with BNU and Bucks Health and Social Care Academy and became a registered independent charity in November 2024. This year, the Buckinghamshire Community Wellbeing Hub in Aylesbury was named Health Creating Community Space/Place of the Year at the 2025 Excellence in Health Creation Award. We look forward to working with the many community support groups and organisations in South Bucks that support health and wellbeing. Local groups are invited to bring residents of all ages to the hub for sessions that will improve their health and wellbeing..
