The hospital site and adjacent clinics also house support staff of the Chalfonts Primary Care Network. These are specialists supporting the work of the Allan, Hall and Misbourne practices close to the Hospital (the Misbourne practice also provides a surgery in Chalfont St Giles). The aim of Primary Care Networks and Community Care Services are to work together to provide neighbourhood health and care services to meet community and personal needs closer to our homes

General Practitioners (GPs) GPs are usually the first point of contact for individuals seeking medical advice, treatment, or a diagnosis. They are highly trained professionals who provide a broad range of services, from managing common illnesses to offering preventative care and coordinating more specialised treatments. While some years ago patients were "on the list" of specific named GPs you are now considered as a patient of the Practice and may be treated by different GPs within the practice or directed to other health professionals working with the practice, but in many cases you may be able to request an appointment with your preferred doctor.
Practice Nurses Work as part of a primary healthcare team with other healthcare professionals including Medical Assistants and Allied Health Professionals in the Primary Care Team. They assist in assessing and monitoring patients with common or ongoing (chronic) conditions and advising patients on their personal health needs.
Primary Care Networks (PCNs) were introduced in 2019 to reinforce GP and Nursing services, integrate primary care with secondary and community services and bridge a gap between GPs and Integrated Care Systems. They are managed by GPs serving 30-50,000 patients, (in our case some 31,000 patients) and led by a GP (currently Dr Ran Hettiaratchi of the Misbourne Practice). The PCN lead plays an important role in coordinating local primary and community health and care services for Neighbouhood Health and Wellbeing. The Chalfonts Primary Care Network employs specialist staff to support Primary Care services including: Paramedics Physiotherapists, Pharmacists, Social Prescriber Link Workers and a Frailty Support Team. Paramedics are trained to assess patients and provide basic treatment. Chalfont PCN GPs may ask a Paramedic to phone or text you to assess the treatment you require. If you feel you need treatment but are unsure, you may wish to call NHS 111 (or contact online) or call your Primary Care Practice.
Physiotherapists a Chalfont PCN GP may ask a “first contact physiotherapist” to assess your need for treatment for problems with your muscles, bones, joints or ligaments. This can then lead to referral for a course of physiotherapy,(as provided at the Centre), advice on self-care or assessment of your fitness for work.
Pharmacists at the Chalfonts PCN monitor the prescriptions of patients and advise on adjustments to improve response to medication. Patients may get a text or phone call from a PCN Pharmacist to advise them of such adjustments. If you feel you have an adverse reaction to your prescribed medicines, you should call your Primary Care Practice.
A Social Prescriber helps patients of the Hall and Allen practices address social concerns that affect their health and helps people join community groups and services that can befriend them and support healthy lifestyles.
A Care Coordinator provides a similar service for patients of the Misbourne Practices.
The Frailty Support Team helps people living in their own homes or care homes who lack mental and/or physical resilience, to recover from events like illness and injury, for example from chronic diseases or falls, the death of a loved one or very complex health conditions . They may require a range of healthcare and social support provided by an integrated team and a community support group. The INT supports patients of both the Chalfonts and the South Bucks Primary Care Networks.
